When applying please bring the following:

Valid Government Issued Photo ID
Valid Saskatchewan Health Card
Long Birth Certificate or Short Birth Certificate & Baptismal Certificate (with parent’s names listed)
Marriage certificate(s) or Change of Name Certificate (used to show name changes)
Family Tree or information to help you fill out a family tree
During registration:
❖ You will completely fill out an application and sign and initial where needed before submitting it.
❖ We will take copies of ID and Documents that you bring in for registration.
❖ A picture will be taken for the card.
Renewal process, for current citizens with expired card photos:
❖ A new photo will be taken for the card.
❖ Copies of valid government photo Id and Saskatchewan health card will be taken.
❖ A copy of your digital signature will be taken to place on the card.
❖ (Only for youth citizens now over 16) A Youth to Adult Form, to document you are okay with the
parent/guardian signing on your behalf on your original application.
